Attention EMS Parents/Guardians and Students,
We are excited to announce that Spring Sports Registration is now open for the upcoming season! To streamline the registration process and better serve our student-athletes, we are utilizing a new system called Final Forms, which has proven to be highly successful at Greenwich High School over the past two years.
If your student athlete is interested in trying out for Boys Baseball, Girls Softball, or Coed Track & Field this spring, please log into Final Forms and register.
The deadline for registering for Spring Sports is March 20, 2024. No registration will be accepted after this date. All students who wish to try out, MUST have a current physical uploaded to Final Forms. The only form accepted is the CT-HAR.
If you have already registered in Final Forms (for the winter season), you MUST log in and select a sport for the spring season. Also, make sure that your student is in the "Green" status. If they are not Green, this means that you or your student is missing something and will not be eligible for tryouts.

FinalForms
The GPS Athletic Department has partnered with FinalForms, an online forms and data management service, to manage athletic participation. All new and returning student-athletes must register with a parent/guardian through FinalForms prior to tryouts or participation.
Click on the Parent Playbook for directions on how to register in FinalForms. If you require any support during the process, scroll to the page bottom in FinalForms and click “Use Support”.
Initial registration will take 15-20 minutes. Registering returning athletes can be completed in 5-10 minutes as FinalForms saves data season-to-season and year-to-year. Parents should have information for physician, dentist, emergency contacts, insurance (if applicable), and a current State of CT Health Assessment Record (HAR) form ready for the registration process. The HAR can be found on GPS Health Forms and Immunizations.
Please register early, as it will take 24-48 hours for the Health Office to process your registration. You may only register for one sport each season. There is no cost to register.
About the CT HAR FORM:
A student athlete must have a current State of CT Health Assessment Record (HAR) form on file with the Eastern Middle School Health Office prior to participation. For sports participation, this form is good for 13 months and must be valid at the time of tryouts and throughout the season. We are no longer using the separate Sports Physical form.
